Sports physicals 




Sports Physical Schedule: 

* Middle school students during school hours on May 31st, 2023
* High school students during school hours on June 1st, 2023

Forms must be returned to the health office by May 26th, 2023 to be scheduled for a school physical.  Any parent/guardian that wishes to be present during the physical, please call Mrs. Boults at 837-6030 ext. 3211 for arrangements at the middle school or Mrs. Kemick at 837-6821 ext 4211 at the high school. 

All private physicals must be dated June 1 or after, per PIAA regulations. Private physicals must be returned to the health office by August 4th in order to participate on the first day of practice for Football and Golf, August 7th, all other sports will start on August 14th.
